Care Coordinator

As a Care Coordinator in the Clinical Services department, you will assist in implementation of customer service programming and revision of Clinical Services policy and procedures.  Telephone communication skills are essential due to extensive interaction with insurance company case managers, hospital discharge planners, and facility personnel.  Competence is demonstrated through data entry using computer software for the purpose of maintaining case records.

Essential responsibilities include, but are not limited to, obtaining member benefits for requested services, accurate data entry of patient encounters into CasePointe system, work in conjunction with hospital discharge planners, insurance company and network facility personnel and MNS Clinical Services staff, assist in maintenance of Clinical Services files, and participates in on-call rotation

Candidates must be able to read, write, and speak the English language in an understandable manner, have excellent verbal and written skills, display a working knowledge of telephone etiquette and deal tactfully with personnel, peers and management

LPN or RN License is required. A Bachelor of Science in Nursing preferred. The ideal candidate will have 2-3 years of experience in case management or related experience and a knowledge of patient assessment, goals setting, evaluation of progress and discharge planning.
